  Nova Scotia Hearing and Speech Centres is committed to providing the highest quality of care to our clients. In order to help us do that, we are asking you to complete this survey about your experiences, so we can continue to do what we do well and to make improvements where needed. These questions relate to the care provided by your audiologist, speech-language pathologist, communication disorders technician and/or administrative support professional.

While we hope that you will give us your feedback, whether you do the survey is entirely your decision. Saying no will not affect your care in any way. You can also skip any question you don’t want to answer and you can stop doing this survey at any time.

The survey will take less than 5 minutes to complete.

Your responses are confidential and will be combined with other responses in a way that makes it impossible to identify individual people. A summary of all results will be made available to clients, families, and staff.

We would ask that the survey be completed by the client or a parent, guardian, substitute decision maker on behalf of the client.
